After a motor failure, purchasing is often asked to find the same kW at the best available price. If the event involved phase loss, that is only part of the replacement decision. The RFQ should record what happened and which parts of the original installation have been checked.
Separate the incident report from the replacement specification
The incident report explains the known fault and outstanding questions. The replacement specification defines what must be supplied. Keep both: an identical nameplate does not show that the original failure cause has been removed, while an incomplete incident report does not establish that a different motor design is required.

Assign responsibility before commissioning
Use records prepared by the service team. Do not ask a procurement officer to perform electrical tests to complete the RFQ.
Do not treat a rotating motor as a healthy motor
After phase loss, a running motor can sometimes continue turning while being exposed to damaging electrical and thermal conditions. A stalled motor can also be damaged by repeated start attempts. Operation should stop pending qualified assessment; the replacement decision should follow that assessment.
Write the scope boundary into the quotation
Ask who supplies the motor, who verifies the existing contactors and connections, who selects or reviews protection, and who authorises restart. Protection products may offer phase-failure functionality, but the exact device and system arrangement need confirmation. A new motor does not validate an old control panel.
An RFQ note for a failure-related replacement
Replacement required after a reported phase-loss event. Attached are the original nameplate and available service findings. Please confirm the proposed motor’s fit and electrical ratings, list any unresolved application assumptions, and identify the checks outside your supply scope before commissioning.
For a Dongchun offer, include output, poles, supply, mounting, duty and the driven machine. Keep the service report with the order so that the installer and supplier work from the same documented condition.
